How Do I Register for Alberta Health Care?

AMA members can come into any AMA centre to register (or update) an Alberta Health Care card at no extra cost.

When you come in, there are some documents you will need to bring:

1) Alberta Health Care Insurance Plan Coverage form that you have filled out

2) Proof you are allowed to live in Canada. You only need to bring in one of these documents that shows your name and birth date:

  • Canadian passport
  • Canadian citizenship card
  • Canadian birth certificate
  • Permanent Resident Card
  • Canada entry document

3) A Photo ID (government issued) document such as your:

  • Canadian/Non-Canadian passport
  • Canadian citizenship card
  • Permanent Resident Card
  • Federal identification card
  • Current Alberta, provincial or territorial driver’s licence

4) Proof that you live in Alberta. Make sure the information you bring has your full name, is dated within 90 days, and has your current Alberta address – this should match the address on your Alberta Health Care form. You can bring one of these:

  • Utility, telephone, gas, or cable TV bills
  • Current bank or credit card statements (originals only)
  • Mortgage documents or residential lease (originals only)
  • Federal income tax return (original only)
  • Alberta social benefits statement or cheque
  • Land title documents (must physically reside at the address)
  • Written confirmation of employment. Must include an employer’s contact name and phone number for verification (dated within 15 days)
  • Written confirmation from an Alberta educational institution that you have dependants attending school

5) Your AMA (Alberta Motor Association), CAA (Canadian Automobile Association) or AAA (American Automobile Association) membership card.
