How Can I Update My Alberta Health Care Information?


If you need to update the information on your Alberta Health Care card like adding a family member or changing your address, we can help when you come into any AMA centre.

If you have need to replace your Alberta Health Care card because you have lost or damaged your card, we make that easy too.

Both services are free for AMA members. What Do I Need To Bring?

  • Print the Notice of Change form. (You can also pick up this form at any of our AMA centres.)
  • Complete the form and bring it into any AMA centre plus the originals of all required documentation.

When Should I Update The Information On My Alberta Health Care Card?

It is important that the information on your Alberta Health Care Insurance plan is always up-to-date so that you continue to receive health care services. Changes in your life that would affect your Alberta Health Care coverage would be for things like:

  • Adding the names of newborns, dependents or of a spouse/partner
  • Cancelling of registration (i.e. due to death, leaving Alberta, etc.)
  • Dependent deletions (i.e. due to death, leaving Alberta, etc.) or reinstatements
  • Non-group Blue Cross
  • Registration reinstatements
  • Address or name change
  • Proof of age for Seniors Blue Cross coverage

Confirmation of student status:

  • Addition of newborns, dependents or a spouse/partner
  • Cancelling registration (i.e. due to death, leaving Alberta, etc.)

How Can I Replace My Alberta Health Care Card?

If you have lost or damaged your card, we can help with that too. Come into any AMA centre and bring your photo ID (so we know it really is you) and one of our agents will be happy to fill out the form requesting a new card on your behalf.

When you come in you will need to provide us with:

  • Your full name
  • Mailing address
  • Telephone number
  • Personal Health Care Number
  • Date of Birth
